Some pets, due to their age and mobility, may require special care. Special care is an extra cost.
For a $10 mandatory fee per animal per stay, all boarding animals 11 years and younger will be covered for up to $300 in veterinary care should a medical problem arise during the boarding stay or for 3 days after the stay. Preexisting conditions are not eligible for coverage by the boarding insurance plan. To avoid any question as to whether a condition was pre-existing, our kennel or technical staff will give each boarding animal a brief exam upon it’s arrival at no charge. If a problem is noticed, the veterinarians will be notified and we will try to reach you so we can determine how you would like to proceed. You will be responsible for the full cost of treating any pre-existing conditions and a veterinary exam. Support staff are not able to diagnose or treat any medical conditions, only bring them to the attention of the veterinarians. The insurance plan only covers veterinary care provided by Germantown Veterinary Clinic, fees from other veterinary clinics will not be covered.
Pets scheduled to be picked up on Sunday need to be picked up between 4pm – 6pm. There is a late fee of $1.00 per minute after 6pm.
Illness or InjuryI acknowledge that in the event of my pet’s illness or injury while in the care of Germantown Veterinary Clinic, Germantown Veterinary Clinic is not liable for such illness or injury. In the event that the staff at the above named veterinary facility are not able to contact me immediately, they are authorized to initiate appropriate treatment until my agent or I can be reached. I agree to pay all related expenses associated with the treatment of my pet until I am available to discuss further care and related fees with the attending veterinarian.
I understand that if I fail to pick up my pet within ten days of notification to the above address, my pet will be considered abandoned and will be handled in accordance with the requirements of state law, and that doing so does not relieve me of my financial obligations to this facility.
In case of illness:
*One of the advantages of boarding your pet at a veterinary hospital is that medical attention is readily available should the need arise. If your pet needs emergency medical attention or becomes ill, you will be responsible for any incurred expenses. Our staff will make every effort to contact you. However, if we cannot contact you, we will proceed with minimal acceptable medical care until you can be reached. You will be responsible for any incurred expenses.
